Please be advised in order to complete a booking you will need your insurance ID card. If you do not have insurance and will be paying out of pocket, you can proceed without. Click here if you want to read more about our Sliding Scale Discount Program to see if you qualify. 

Please follow the prompts in the scheduler until the thank you page. Once your appointment is submitted, someone will be in touch within 48 hours to send a link to our Patient Portal where you will be required to complete all intake forms 24 hours prior to your scheduled appointment. 

Note: all appointments are booked with at least 3 days advance notice to give our team time to verify all documentation has been completed prior to your scheduled appointment. Failure to complete the required forms 24 hours before your appointment will lead to an automatic cancelation and an email inviting you to reschedule.
