Please be advised in order to complete a booking you will need your insurance ID card. If you do not have insurance and will be paying out of pocket, you can proceed without. Click here if you want to read more about our Sliding Scale Discount Program to see if you qualify. 

Please fill out the following form. Once you submit it, someone will be in touch within 48 hours to send a link to our Patient Portal where you will be required to complete all intake forms 24 hours prior to your scheduled appointment. Note: Failure to complete the required forms 24 hours before your appointment will lead to an automatic cancelation.
